Step 1: Duplicate our Tally form template

Step 2: Personalize your Tally form


As we mentioned in our email, this form contains 2 questions. And depending on the “rating link” your reader clicks on, they will be asked just one or both questions (and the ask will have a slightly different language).

Luckily, you don’t have to worry about setting up the form conditional logic because we already baked in the form template!

We do encourage you, however, to review and tweak the language we used for the copy so it sounds like you and it fits the specific constraints of your newsletter.

Make sure to also change the title of the form!
